Membership application

The Crestwood Neighborhood Association is dedicated to the preservation of our community and the well-being of its residents. You and your family are invited to join us as we address the issues that concern us here in Crestwood. The only requirement is that you reside within the boundaries of Crestwood.

Meetings are the first Monday of the month at Central Christian Church, 3205 Hamilton Ave. Active committees are working on everything from real estate taxes, public health and safety, environmental, traffic and construction issues to beautification. Please consider joining one of our committees, and contact us with your suggestions of items to be included in the next open meeting agenda.

Membership dues are $12 per year per household. Please make checks payable to The Crestwood Neighborhood Association and return the form to us at a meeting or at the address below the form.
